Thank you for choosing Fox Valley Women’s Healthcare for your healthcare needs. As an office, one of our goals is to educate our patients so that they may make well informed decisions in regards to their care.
We would like you to fill out the attached "patient profile form" and bring it with you on the day of your appointment. Filling these out prior to your appointment and bringing them with you will enable us to see you more quickly. You can also click on our link to our EMR (Electronic Medical Records) system, digiChart, to enter your healthcare information.
Please, bring your insurance card to your visits so that it may be copied and kept on file. If you do not bring a current insurance card with you at the time of your visit, you may be asked to reschedule.
Please check to see that your insurance covers preventive medicine or well woman examinations. If you have no medical complaint when you are seen, your examination will be coded as a yearly preventive medicine examination and will be submitted to your insurance as such. If you do not have benefits to cover preventive medicine, you will be solely responsible for the health care providers charges as well as the reference lab charges for your pap smear.
We make every effort to keep down the cost of your health care. Therefore, payment for services is expected at the time of service for all office care. HMO and PPO members should know their co-payment or deductible requirements for the office visit. It is your responsibility to know what your co-payment or deductible is, as deductibles and co payments vary from company to company. Along with cash and personal checks, MasterCard, Visa and Discover is accepted. We do not accept debit cards.
