Electronic Medical Records
Our office uses a computer-based method to store medical records and charts. It is recommended that each patient fill in a medical history online before your visit.
You should have received a unique username and password from our office that you will use to log in. If you haven't received this information, please call the office. Before you come to our office for your scheduled appointment, we would like you to visit https://patients.digichart.com to fill out a detailed medical history. This should take 20-30 minutes. By completing the history online, you will save time at your next visit.
The information you provide is secure and protected, and complies with all federal and HIPPA regulations. No one outside of our practice can access our patients' records.
It is very helpful in completing this on-line history prior to your appointment.
